JOB TITLE: SOCIAL WORKER - LMSW

DIVISION: SERVICES

FLSA: EXEMPT

LOCATION: BROOKLYN, NY

SUMMARY: The LMSW is responsible for coordinating outpatient and inpatient mental health and substance abuse services for a 200+ bed men’s homeless shelter. Proof of COVID-19 vaccination is required. Responsibilities Manage a caseload of high-risk families with children. Facilitate mental health/substance abuse workshops and onsite support groups for clients. Utilize knowledge of community resources to make appropriate referrals and coordinate services. Assess clients’ psychosocial functioning independently to develop and implement treatment plans. Conduct psychosocial assessments and provide treatment from a strength-based, client-centered perspective. Apply knowledge of medical and mental health diagnoses, disabilities, and treatment procedures. Provide consultation to staff regarding clients’ psychosocial needs and health care compliance. Use computer software for documentation, data management, and tracking. Provide crisis intervention and stabilization; may require after-hours or weekend follow-up. Maintain and review documentation and reports timely. Identify program needs and recommend improvements. Perform related tasks as assigned. Minimum Requirements LMSW certification, a master’s degree in social work from an accredited CSWE program, and at least two years of specialized social work experience.
